If this is a manual process for you, it probably goes a lot like this. You open up a spreadsheet, create a new file, and insert several columns in it, such as "Keyword/Keyphrase", "Last Week's Google Ranking" and "This Week's Google Ranking".
Then you go ahead and visit Google, input your keyword, hit Return and then scroll down in the displayed results in search of your site. (If your site is #1 — lucky you! You don't have to scroll down, but that's beside the point.)
